Please understand that no child will be able to participate in our program until this waiver is signed, dated, and returned. Thank you for your cooperation.

I am giving my child permission to participate in EmpowerMovement Education's dance program. I understand that my child is participating in a program that uses physical activity. I have agreed that my child is in good physical condition and does not suffer from any condition that would prevent or limit participation in the program. I understand that I am to assume full responsibility for all risks of physical injury for participation in EmpowerMovement Education's program. I am aware that this is a release of liability and understand all risks to my child in connection with any instruction and further release EmpowerMovement Education and its employees from liability for any injury or illness sustained by my child while he/she/they is enrolled in the curriculum program with EmpowerMovement Education.

I waive all rights and/or causes of legal action of any kind against EmpowerMovement Education or any of its employees. This includes all liabilities, claims, costs, expenses, judgments, attorney's fees or court costs for any occurrences in connection with EmpowerMovement Education. I understand that EmpowerMovement Education is not responsible for any child under my supervision who is left unsupervised in any outside area near the school. I understand that EmpowerMovement Education will only supervise my child/children when he/she/they is participating in scheduled dance instructions, activities, or programs.

I understand that EmpowerMovement Education is not responsible for personal property that is lost, damaged, or stolen while my child is participating in the program. I am of lawful age and competent to sign this waiver. My signature below states that I acknowledge and understand all of the contents of this release form.
